These forms can be printed and mailed directly to the Office of the City Clerk.

PICK UP APPLICATION FORMS

Voter registration applications are available at the following locations:

  • Most State of Hawaii government agencies (voter registration only)
  • Public Libraries
  • State of Hawaii Tax Booklet

With the exception of drivers licensing agencies (that incorporate a voter registration form as part of the drivers license application), locations listed above will not collect completed application forms.

Please mail completed forms directly to the Office of the City Clerk. At 530 S. King St. Room 100 Honolulu, HI 96813.

CANCEL YOUR REGISTRATION

If you no longer wish to be a registered voter, you may voluntarily cancel your voter registration. You may also cancel the voter registration of a deceased relative. The CANCELLATION FORM may be submitted to the Office of the City Clerk.

NOTICE OF VOTER REGISTRATION

If you are properly registered to vote, you will receive a Notice of Voter Registration card via mail in April during scheduled election years. This card is also periodically dispatched to newly registered and re-registered voters throughout the election year.

SIGNATURE UPDATES

When voting by mail, your signature is required. Election officials will compare the signature you provide with a signature that we have on file. If you want to provide us with an updated signature, please complete and return a new voter registration application and submit it to the Office of the City Clerk.
