How to Obtain a Golf I.D. Card

Qualification:

Golf I.D. cards are issued only to bona fide Hawaii residents.

Documents Required to Obtain a Golf ID Card:

Valid Hawaii Driver’s License*

The Hawaii State I.D. Card* IS accepted in lieu of a Hawaii State Driver’s License, providing the card was issued after January 1, 2013 AND identifies Hawaii as the card holder’s Primary Domicile.
 
 

OR one of the following:

A

A picture ID AND one of the below:

  1. Previous year’s Hawaii State income tax return with W-2 form.**
  2. Filed current year Hawaii State income tax return with W-2 form.**
  3. Hawaii voter registration certificate.
  4. Current real estate property tax assessment document for your Hawaii residential property. (Applicant must be listed as an owner on assessment)
  5. Homeowner’s insurance policy for your Hawaii residence.
    (Applicant’s name must be shown on the Policy and must show proof of payment)
  6. An approved Hawaii State Tax Clearance Certificate (Form A-6) from the Hawaii State Department of Taxation (Applies to those individuals who don’t have a copy of their Hawaii State Tax return or are new filers, who have spent the previous year residing in Hawaii).

**Note:  Person must be filing as a Resident of Hawaii; Form N-11.

B

Student ID (Dependents ages 8-17 years old)
(Any adult or Minor under the age of 18, who are claimed as dependents by persons who are not Residents of Hawaii, do not qualify) (Children below the age of 8 can not obtain a Golf ID card)

  1. (Local)-Student picture ID card with a document reflecting their date of birth OR
  2. (Local)-Student ID card with a document having a photo and date of birth.
  3. For those students under 15 who do not have the above documents, a Golf ID card will be issued based upon the following:
    • The student is accompanied by his/her parent; AND
    • The student’s parent meets the criteria for a Golf ID card and presents the student’s birth  certificate.

C

U.S. military/dependent ID card (non-retiree) with matching form of identification which has a Hawaii address (i.e. Current Bank Statement, Recent Telephone or Utility Statement with the applicant’s name on the account).  Hawaii Resident Golf I.D. card will be valid for the duration of Current Service Deployment to Hawaii.
