Applicants under 18 who do not have two proof of address documents may use documents presented by their parents.<\/li><li>Applicants under 18 must submit the <a href=\"https:\/\/www4.honolulu.gov\/docushare\/dsweb\/Get\/Document-325983\/Parental%20Consent%20Affidavit.pdf\">Parental Consent Form (CS-L (DL) 239)<\/a>. Both parents listed on the birth certificate <strong>must<\/strong> submit a parental consent form.<\/li><li>If one or both parents are not able to accompany you to your appointment, the parental consent form of the absent parent(s) must be notarized by a notary public and brought with you to your appointment.
